A pretreatment estimate is provided to all of our patients at the consultation before treatment is rendered. The estimated amount will vary depending on your insurance benefits quoted to our office based on plan maximums, copayments, and deductibles. This is an estimate and not a guarantee of payment by your insurance company. Most insurance companies will respond within 4-6 weeks. Any remaining balance, after your insurance has paid, is your responsibility. Not all services are a covered benefit in all insurance contracts. Any service not payable by your insurance carrier will require full fee payment at the time of service. If you have any questions regarding your insurance coverage, please contact your insurance carrier.

Payment for all estimated out-of-pocket expenses is expected at the time of service unless other arrangements have been made in advance. For your convenience we accept MasterCard, Visa, FSA/HSA, Personal Check, and Cash.
